Access a support plan

How a support worker or trustee opens and works with a beneficiary's support plan.

A support plan is the beneficiary's record of their goals, preferences, services, and appointments. Examples of preferences are accessibility needs such as braille. Examples of services are personal care.

You can open a support plan only when two things are true. First, you hold an active relationship with the beneficiary. A support worker holds a support arrangement. A trustee holds a delegation. Second, the beneficiary has granted you consent to the plan.

Consent is never implied. An active arrangement or delegation alone gives no access.

Purpose

This guide helps a support worker or trustee open and work with a beneficiary's support plan. After following it, you will know how to use the plan to support the beneficiary well.

Who this is for

This guide is for a support worker who has an active arrangement, and for a trustee who has an active delegation and consent.

What you need

  • Your Community OS identity. Read the guide to create your identity first if you do not have one.
  • An active arrangement or delegation for the beneficiary.
  • The beneficiary's active consent to the support plan.
  • Sign in to Community OS.

What you will do

  1. Open the support plan.
  2. Read the plan details.
  3. Update the plan if you have write permission.
  4. Work within your limits.

Instructions

  1. Sign in to Community OS.
  2. Open the beneficiary's support plan.
  3. Read the title. It names the plan.
  4. Read the goals. These are what the beneficiary wants to achieve.
  5. Read the preferences. These are how the beneficiary wants support delivered. For example, braille for written material.
  6. Read the services. These are the support services arranged for the beneficiary.
  7. Read the appointments. These are the planned appointments with the beneficiary.

If you have write permission, you can change the plan.

  1. Select Edit plan.
  2. Change the sections you are allowed to change.
  3. Review the changes.
  4. Select Save.

Always follow the plan. Ask the beneficiary or the coordinator before you change anything you are not sure about.

What happens next

  • Your access continues while your arrangement or delegation is active and the consent stays granted.
  • Changes you make are saved to the plan. The beneficiary can review them.
  • When the arrangement ends or the consent is revoked, you lose access. Status for the plan remains Active until the beneficiary decides otherwise.

Troubleshooting

  • Problem: you cannot open the plan. Fix: check your arrangement or delegation is active and within its dates. Then check the consent is active and has not expired.
  • Problem: the consent was revoked but you still need access. Fix: ask the beneficiary to grant consent again. Only the beneficiary can grant consent.
  • Problem: you can open the plan but not change it. Fix: you have read permission only. Ask the beneficiary to grant write permission if you need to change the plan.
  • Problem: the plan looks out of date. Fix: contact the coordinator. Do not change details you are unsure about.
