Track a referral
Record a referral, choose a provider, and follow up until the person gets the help they need.

Purpose
A referral connects a person with the services they need. This guide shows you how to record a referral, choose a provider, update its status, and record the outcome so nothing is left hanging.
Who this is for
This guide is for support workers and coordinators who refer people to services.
What you need
- The person's consent to make the referral.
- A record of what the person needs.
- A provider to refer them to.
- Access to the referrals list.
What you will do
- Create the referral.
- Choose a provider.
- Set the status.
- Follow up with the provider.
- Record the outcome.
Instructions
- Open the Referrals menu from the main navigation.
- Select New referral.
- Select the person being referred in the Referred person field.
- Select the person making the referral in the Referrer person field.
- Add what the person needs in the Needs field.
- Select the provider in the Recommended provider field.
- Set the Status.
- Select Save.
To follow up:
- Open the referral record.
- Select Follow up.
- Contact the provider and confirm they received the referral.
- Confirm the person has been seen or is on a list.
- Update the status if it changed.
To record the outcome:
- Open the referral record.
- Select Add outcome.
- Describe the result in the Outcome field.
- Set whether follow-up is needed in the Follow-up required field.
- Add any notes.
- Select Save outcome.
What happens next
The referral shows its status and outcome. If follow-up is required, the record keeps that flag so the support worker knows to check again. The person's needs stay linked to the referral for reporting.
Troubleshooting
| Problem | What to do |

| The provider has not responded | Follow up with the provider and note the contact attempt. |
| The person's needs changed | Open the referral, edit the Needs field, and save. |
| The referral is no longer needed | Update the status to reflect that the referral stopped, and add a note. |
| Follow-up is overdue | Open the record, check the follow-up flag, and contact the provider. |
